Insurance
 

We accept the insurance companies listed below.  In some cases, the office you visit will not accept your insurance, so check with us first.

In case your insurance is not listed, you are still encouraged to contact us to see if it has been added, or if we can provide alternative services.  Your carrier may require periodic authorization.

For Urgent care patient, if we accept your Insurance carrier, but you are registered with another doctor office and your doctor can not see you quickly enough, stop by after 5 PM or on the weekend and we will be able to see you under your insurance coverage.
The following is required from all Insurance Patients:
  1. insurance card,
  2. driver's license or official photo ID
  3. Any important medical information.
Please be sure to let our staff know of ANY allergies, current medications and your medical history.

Note: PPO patients will require periodic authorization which may not be available on weekends and holidays. If you were seen at our facilities under your insurance plan within the calendar months, such authorization may not be required
